Following your initial conversation, Kristine will mail you a New Patient Folder that contains more information about our practice and the forms you’ll need to fill out for your first appointment. If you have any questions about the forms or need help filling them out, we are happy to assist.

When You Arrive

Upon arrival please be sure to check in with the front desk and give your completed forms and other requested documents to the front desk scheduler. If you still have incomplete information in your new patient packet, please let staff know at this time so that they may help you complete it. You will be given a privacy policy form and a medication agreement form to read and sign. Please be prepared to pay your copay at this time.

4

In the Exam Room

One of our friendly Medical Assistants will lead you to the exam room. They will take your vitals and discuss your reason for the visit.
